Louisiana Policy Issuance: Uninsured/Underinsured Motorist (UM/UIM) Form Requirements

When placing a policy in Louisiana, state compliance regulations require the insured to electronically sign a specific Uninsured/Underinsured Motorist Bodily Injury Coverage Form.

This form is a state-mandated waiver required specifically when you select Uninsured/Underinsured Motorist (UM/UIM) limits that are lower than the policy's Bodily Injury Liability limits (effectively "waiving" the excess coverage). For example, if you bind a $2 million Umbrella but only select $1 million in UM/UIM coverage, the insured must sign this form; however, if you select matching limits of $1 million for both Umbrella and UM/UIM, no signature is required.


If this coverage is waived (limits are unequal), the policy cannot be bound or issued until the insured has electronically signed the required document.


How It Works

The system will automatically detect if the waiver applies based on the limits you selected. Regardless of how payment is processed, the insured must complete this signature via an email link sent by the system.

There are two workflows depending on how payment is handled:

1. Entering Payment Information on Behalf of the Insured

When you enter the payment details on behalf of the insured:

  1. The system will generate an email to the insured containing a link to the form.

  2. The policy is NOT bound immediately.

  3. The insured must click the link and electronically sign the document.

  4. Once the signature is captured, the policy will be successfully bound and issued.

2. If You Send a Payment Request to the Insured

When you send the payment link to the insured:

  1. The insured receives the link to complete the process.

  2. Step 1: They will be prompted to electronically sign the UM/UIM waiver document.

  3. Step 2: After signing, they will be prompted to enter their payment information.

  4. The policy is bound and issued only after both the document is signed AND payment is successfully entered.
